2. Enter your email address, your Patient ID (your account # on your statement), your last name, and your zip code. Click on “PAY NOW”.

3. Add your first name and click on “NEXT”.

4. Click on “CONTINUE AS GUEST” at the bottom of the page.

5. Enter your payment info, and click on “NEXT”.

6. Review and click on “CONFIRM”.

Open the payment portal

Head to pay.instamed.com/ SWANPRIMARYCARE2 in a new tab.

Step 2:

Verify your details

Enter your email, Patient ID (on your statement), last name, and ZIP. Click on “PAY NOW”.

Step 3:

Add your first name

Type your first name and select “NEXT” to continue.

Step 4:

Continue as guest

Choose “Continue as guest” at the bottom no account needed.

Step 5:

Enter payment info

Add your card or bank details and select “NEXT”.

Step 6:

Review & confirm

Double-check the summary and hit “CONFIRM”. You’re done.
