Can I use public transportation to get to the office/hospital?

It is the student’s responsibility to research which public or private transportation options are available.

Where should I go on my first day?
Please report to the following address on your first day: 
303 E Army Trail Rd, Suite 111, Bloomingdale, IL 60108 at 10 a.m. (on Monday) or 2p.m.(on Tuesday – Thursday).
What will my schedule be like?

Your schedule will be discussed during your orientation on the first day.

Can I request days off?

Yes, days off can be requested in accordance with our leave policy.

Are there designated lunch breaks? Can I use a microwave?

Yes, there are designated breaks for meals. Facilities including a microwave and a coffee maker are available for use.

Do I have to come in on weekends?

Weekend attendance is not mandatory unless specified by your supervisor.

Is there a dress code?

The dress code is business casual, supplemented by a white lab coat.

Is there EMR access at the clinic?

Yes, EMR access is provided at the clinic.

How can I request my Letter of Recommendation (LOR)?

You can submit your request for a Letter of Recommendation via email at least 2 week in advance of deadline to education@swanprimarycare.com
