Wahaj Ahmed MD
Dr. Ahmed received his medical degree from Windsor University School of Medicine, and completed his internal medicine residency at Mount Sinai Hospital in Chicago. As a board-certified internist, Dr. Ahmed has been practicing medicine for the past five years. While at AMITA GlenOaks Hospital, Dr. Ahmed helped the hospitalist program achieve the most improved in terms of quality. Due to his past research experience at University of Chicago, Dr. Ahmed also understands the importance of remaining up-to-date with new guidelines in patient care. This allows him to uniquely bond with his patients to positively change their health outcomes for the better.
Sunnan Moinuddin MD
Dr. Moinuddin transitioned into medicine after a career in engineering, driven by a deep passion for healthcare and a commitment to making a meaningful impact on patients’ lives. Having witnessed the challenges of fragmented healthcare, he is dedicated to improving care coordination and ensuring patients receive seamless, high-quality treatment across all settings.
Abdullah Z. Farooqui MD
Dr. Abdullah Z. Farooqui is a certified hospitalist with advanced training in Family Medicine and a strong commitment to clinical precision and compassionate care. He earned his medical degree from Windsor University School of Medicine and completed his residency at Jackson Park Hospital’s Family Medicine Program, where he developed a methodical, evidence-based approach to inpatient care.

Rohini Attavane, FNP, brings a wealth of experience in primary care, internal medicine, and critical care. She earned her Master of Science in Nursing with a Family Nurse Practitioner (FNP) specialization from Chamberlain University in 2017 and a Bachelor of Science in Nursing from the College of Nursing, Bangalore, India, in 1990.
Annmary Kachappilly, FNP
Annmary Kachappilly, FNP, is a board-certified Family Nurse Practitioner dedicated to providing compassionate and holistic patient care. She earned her Master of Science in Nursing with a Family Nurse Practitioner specialization from Chamberlain University in December 2021 and her Bachelor of Science in Nursing from Chamberlain College of Nursing in Addison, IL, in April 2016.

Rashidah Sembatya, FNP-C, is a board-certified Family Nurse Practitioner dedicated to providing comprehensive and compassionate care to patients of all ages. She earned her Master of Science in Nursing from Chamberlain University and her Bachelor of Science in Nursing from Grand Canyon University. She completed her clinical training in Family Medicine with Family Medicine Specialist Inc., further refining her skills in primary care.

Sakeenah Latifi is a board-certified Physician Assistant dedicated to long-term wellness for the patients and families she serves. She earned her Master of Medical Sciences in Physician Assistant Studies from Midwestern University and brings a compassionate, whole-person approach to care.
Kristine Bautista
Kristine Bautista is a board-certified Nurse Practitioner currently practicing as a hospitalist at Glen Oaks Hospital, Alexian Brothers Hospital, and Gottlieb Memorial Hospital. She brings over 20 years of healthcare experience, having spent her career at UChicago Medicine. As a registered nurse, she focused on oncology care before advancing her practice as a Nurse Practitioner specializing in advanced heart failure and heart transplantation.

Nadia Rahman, PA-C, is a board-certified Physician Associate with a clinical focus in Internal Medicine. She earned her Bachelor of Science from Wayne State University, where she developed a deep interest in public health and the social determinants of health, particularly within underserved communities. She later completed her Master of Physician Assistant Studies at Chamberlain University. During PA school, multiple Internal Medicine rotations solidified her decision to pursue the specialty.

Aaron Go is a nurse practitioner at Swan Primary Care, bringing a decade of ICU nursing experience to his work with patients. He earned his Bachelor of Science in Nursing from the University of Illinois at Chicago in 2014, then spent the next several years in critical care, first as an ICU nurse at St. Alexius Medical Center from 2015 to 2022, then at AdventHealth Glen Oaks Hospital from 2022 to 2026. That background shapes how he approaches care today: attentive, thorough, and quick to catch details that matter.
