Annmary Kachappilly, FNP

Annmary Kachappilly, FNP, is a board-certified Family Nurse Practitioner dedicated to providing compassionate and holistic patient care. She earned her Master of Science in Nursing with a Family Nurse Practitioner specialization from Chamberlain University in December 2021 and her Bachelor of Science in Nursing from Chamberlain College of Nursing in Addison, IL, in April 2016.

With nearly two years of experience at Swan Primary Care, Annmary specializes in chronic disease management within subacute rehab settings. Her background includes extensive hospital experience, having worked as an ICU nurse at Glen Oaks Hospital for three years and in the med-surg/telemetry unit for four years. Additionally, she has served in multiple nursing homes, gaining valuable expertise in geriatric and rehabilitative care.

Annmary is deeply committed to making a difference in her patients’ lives. She embraces a holistic approach to healthcare, focusing on health promotion and improving patient outcomes. She is fluent in English and Malayalam, enabling her to connect with a diverse patient population.

As a board-certified Family Nurse Practitioner by the American Academy of Nurse Practitioners, Annmary remains dedicated to continuous learning and professional excellence. Outside of work, she enjoys engaging in community health initiatives and spending quality time with family.
