Wahaj Ahmed MD
Dr. Ahmed received his medical degree from Windsor University School of Medicine, and completed his internal medicine residency at Mount Sinai Hospital in Chicago. As a board-certified internist, Dr. Ahmed has been practicing medicine for the past five years. While at AMITA GlenOaks Hospital, Dr. Ahmed helped the hospitalist program achieve the most improved in terms of quality. Due to his past research experience at University of Chicago, Dr. Ahmed also understands the importance of remaining up-to-date with new guidelines in patient care. This allows him to uniquely bond with his patients to positively change their health outcomes for the better.
Dr. Ahmed realized the difficulties in patient care with multiple providers handling care for one patient overtime. As a founding member of SWAN Primary Care, Dr. Ahmed is able to directly manage care for his patients no matter where they are.
Sunnan Moinuddin MD
Dr. Moinuddin transitioned into medicine after a career in engineering, driven by a deep passion for healthcare and a commitment to making a meaningful impact on patients’ lives. Having witnessed the challenges of fragmented healthcare, he is dedicated to improving care coordination and ensuring patients receive seamless, high-quality treatment across all settings.
As the Chairman of Medicine at AdventHealth GlenOaks Hospital, Dr. Moinuddin plays a vital role in shaping medical practices, enhancing patient outcomes, and leading quality improvement initiatives. His approach to medicine focuses on building long-term relationships with patients, providing continuity of care in the clinic, hospital, and nursing home. He is a strong advocate for his patients, ensuring they receive the comprehensive and compassionate care they need and deserve.
Abdullah Z. Farooqui MD
Dr. Abdullah Z. Farooqui is a certified hospitalist with advanced training in Family Medicine and a strong commitment to clinical precision and compassionate care. He earned his medical degree from Windsor University School of Medicine and completed his residency at Jackson Park Hospital’s Family Medicine Program, where he developed a methodical, evidence-based approach to inpatient care.
Recognized for his disciplined work ethic and thoughtful patient engagement, Dr. Farooqui brings a high level of clinical proficiency to manage complex medical conditions with clarity, confidence, and coordination. He prioritizes continuity of care, empowering patients and families through open communication.
Fluent in English, Hindi, and Urdu, Dr. Farooqui offers culturally competent care to a diverse patient base. He is a diplomate of the American Board of Family Medicine (ABFM) and an active member of the American Academy of Family Physicians (AAFP).
Whether leading a multidisciplinary team or guiding individual recovery, Dr. Farooqui is committed to delivering a seamless hospital experience to patients. His goal is to not only treat illness but to elevate care with integrity, attentiveness, and unwavering clinical excellence.
